Support coordination helps you to get the best and most relevant supports accessible through your NDIS plan.  

 

We have also helped many improve and increase their NDIS funding by at least 50%, so you or your loved ones can get further access to supports you need.

Support coordination is a Capacity Building support that primarily seeks to help you:

​

• Understand and use your NDIS plan to pursue your and achieve personal goals

• Connect you with NDIS and Independant providers, community, mainstream and other government services that are available and relevant to you (that may not have even realised).

• Build your independance, confidence and skills to use and coordinate your supports.

• Keep track of your NDIS budget and timeline, and lodge funding reviews to the NDIS within  appropriate time frames so your lifestyle is not affected by interuption of supports.

 

Shadow Support Services delivers two levels of coordination in line with the NDIS:

1. Support connection – We will help you to build your ability to connect with informal, community and funded supports enabling you to get the most out of your plan and pursue and achieve your goals.
    

2. Support coordination – coordination of supports: This support will assist you to build the skills you need to understand and use your plan. A support coordinator will work with you to ensure a mix of supports are used to increase your capacity to maintain relationships, manage service delivery tasks, live more independently and be included in your community.

 

If you have funding for Specialist Support Coordination or Psychosocial Coaching

Shadow can connect you with an accredited provider and work along side of you to access these. This level of coordination It is for people whose situations are more complex. A specialist Support Coordinator will assist you to manage challenges in your support environment and ensuring consistent delivery of service.

You may also wish to focus on a specific goal in your NDIS plan, such as those related to finding suitable Home and Living supports. You can use your support coordination funding to work with Shadow, who has the skills, knowledge and experience to meet your specific needs.
